Purpose
To provide an idea and a flexible template for an open source, web based, COVID-19 symptom tracker!
You can also find a sample "API" written in PHP that would need to be development by you.
History
We developed this app early on the 2020 COVID-19 pandemic to help Wartburg College identify constituents that required isolation and treatment.
The idea behind the app was to make something adaptable and integrated to Wartburg's ERP, so we built all of the original backend in the DotNetNuke framework, and highly connected to their JEnzabar SIS.
A weekly email with warnings to fill out the app were also sent to those who didn't fillout the app!
Because of the nature of the project, most of the backend is propriatary, but the front end is free for grabs! Checkout our source code page!

This Covid-19 screening app only provides the UI so you can connect with your own back end.
We have provided a php file with a sample of what the API should respond, as well as a sample of the databass schema.
